Before applying, please ensure:

  • you are aged 18 years and over and under 56
  • you are an Australian citizen or permanent resident of Australia

To complete the application form you will require:

  • credit card number and expiry date; or
  • bank account number, name and the BSB number; and
  • details of your medical history on hand.
